CALVARIAL BONE GRAFTING: THE VERSATILE CLINICAL APPLICATION IN CRANIOFACIAL RECONSTRUCTION

Abstract
Bony defect of the cranium or face has been reconstructed with alloplastic material and autogenous bone or cartilage graft. Iliac, costal, cranial and tibial bone have been used for the reconstruction of the craniofacial bony defect as a autogenous bone graft. Each of these methods has disadvantages, but the calvarial bone graft is most trouble free, and has many advantages compared with the traditional other bone grafts. Between March, 1984 and August, 1985, 12 patients with craniofacial bony defect have been treated with calvarial bone grafting. A total of 13 cases in 12 patients have undergone reconstruction of deformities and defects resulting from acute trauma, sequelae of trauma, congenital anomalies, and tumor resection. Apart from 1 cranial reconstruction, 12 cases were all facial reconstruction. With a high speed drill and an osteotome, outer cranial table parietal bone was harvested in 12 cases, and outer cranial table frontal bone above hair line in 1 case. There have been no complications in 13 cases. After postoperative follow-up between 1 month and 17 months, the improvements had been maintained clinically and a serial skull X-ray identified persistence of the implanted bone.
